Reporting Your Job

Local 491 depends on the participation of all members to alert us of new projects in town, as some projects may attempt to work in our jurisdiction without ever contacting the local. If we don’t know about the project, we cannot be there to represent you.

This form is completely anonymous unless you opt to include your contact information (it’s your choice).

Request Permission
to Work Here

You must obtain permission to work in the jurisdiction if you are not a 491 member.  Please see The International Constitution and By-Laws, Article 21, Section 7.

If you are an IATSE member, but not a member of Local 491, and you’ve been contacted by a production to work within the jurisdiction of Local 491, you are required to obtain permission, in writing, from the host Local (491).

Permission to work may be obtained from Darla McGlamery.

Two documents are needed for permission:

  • Dues Deduction Form: Assessment Authorization
  • Written request for permission to work in 491

Authorization for Represntation

INTERNATIONAL ALLIANCE OF THEATRICAL STAGE EMPLOYES, MOVING PICTURE TECHNICIANS, ARTISTS AND ALLIED CRAFTS OF THE UNITED STATES ITS TERRITORIES AND CANADA, AFL-CIO-CLC [IATSE]

Designation of Collective Bargaining Representation and Application for Membership

I hereby authorize International Alliance of Theatrical Stage Employes, Moving Picture Technicians, Artists and Allied Crafts of the United States its Territories and Canada, AFL-CIO, CLC [IATSE] and Local No. 491, IATSE to represent me for the purposes of collective bargaining in matters of wages, hours, and other terms and conditions of employment with my current employer as set forth below as well as all other employers for whom I may become employed after the date set forth below on all present and future job sites. I understand this authorization card may be used to obtain recognition from my current or future employer without election. I further understand that this authorization shall not expire until such time as I revoke it in writing.

Joining 491

Joining Local 491 is a multi-step process.

After your initial inquiry, a packet will be sent with info about Local and next steps. You will need to return the following:

  • New Applicant Letter signed
  • Rights and Responsibilities Acknowledgement signed
  • Copy of DL as proof of residency
  • Resume
  • Letter of intent (why you want to join)
  • Completed Sponsor Form

Once received and reviewed, you will be contacted to schedule an interview with our Business Agent (in-person or virtually).

After interview, a staff member will review the application packet with you and then send it along.  Packet will include:

  • Membership Information form
  • 2x Reference Form – you will need two different references
  • Obligation
  • Payroll Deduction Authorization
  • Authorization for Representation
  • IATSE Application
  • Back-2-One PAC form (voluntary)
  • ASA Safety Pass Application
  • Voter Registration Form (if unregistered)

The above forms need to be returned completed in their entirety along with any certifications and/or license copies required to support your craft listing.

When Membership Application Packet returned, the ASA Safety Pass app sent in and PIN issued directly to you.  We will keep track of your class completions.

Once all steps, forms, requirements (such as sponsor and reference forms) are completed, then payment will be secured  and your application will be processed by our Secretary-Treasurer and sent on to IA’s General Office for approval.

The whole process can take from a few weeks to many months depending upon your level of commitment to completing the process diligently.
